Health Informatics Consulting and The New Jersey Academy of Family Physicians have partnered in providing Patient-Centered Medical Home Recognition Services and Training.
The Patient Centered Medical Home (PCMH) is a model for delivering comprehensive, coordinated, patient-centered primary care that also recognizes the need for enhanced payment for primary care services. We will provide our expert services to help you meet the "joint principles" of the National Committee for Quality Assurance (NCQA) PCMH model.

- The AAFP, AAP, ACP, and AOA recognize and support the use of PCMH within medical home demonstration projects
- Where select demonstration programs are in place, practices earning PCMH Recognition may qualify for additional payment

Our flat-rate service package will assist you in building your medical home through:

HIC and NJAFP will guide you through the rigorous and comprehensive review that ensures your practice meets the well-managed, high-quality care and services that the NCQA symbol represents. We will assist you in the accreditation process and implementation of the nine standards that support the "joint principles," and provide a more practical description of what constitutes a Patient Centered Medical Home.
